Easy to Refer. Easy to Collaborate With.

For referral partners

A clear path for the cases that need continuity most.

You know the families who have been handed off one too many times—where every transition means a new agency, a new waitlist, and progress that collapses in the gap. Regional Centers, schools, health plans, and discharge planners refer to Aspire for exactly those situations: when continuity across settings is what stands between stability and the next crisis. We keep referrals straightforward, keep you informed, and once a family is with us, the starting-over stops.

When to Refer

Consider Aspire when continuity is at risk

Aspire is built for complexity and continuity—not routine, early-intervention, or high-volume cases. These are common signals that a referral may be appropriate.

Provider churn or repeated discharge

The case has moved between agencies or repeatedly lost services, and it needs to stop starting over.

Severe or high-risk behavior

Aggression, self-injury, property destruction, or elopement is present and needs experienced clinical support.

Hospital or restrictive-placement step-down

A family is returning from a hospital or more restrictive setting, and the transition has to hold this time.

Needs span multiple settings

Support crosses home, school, community, and program settings, and coordination keeps breaking down.

A high-risk transition is ahead

Adolescence or the move from school to adult services is approaching, with a great deal riding on continuity.

The caregiver or team is stretched thin

The plan needs more structure, coordination, and reliable follow-through around the people carrying it.

Complexity Is Not Always Loud

The quieter end may still need real expertise

Aspire also serves needs such as selective mutism and low-rate but high-stakes behavior. A case does not need to look visibly chaotic to require specialized judgment and continuity. If you are not sure whether a case is a fit, send it anyway. We will help identify the right next step, even when that step is someone other than Aspire.

How Collaboration Works

Clear from referral through onboarding

Partners should not have to chase a referral, wonder whether it was received, or lose sight of what happens next. Aspire keeps the path visible while respecting the larger system around the family.

One Clear Entry Point

Straightforward to refer

One referral, one point of contact. We do not make you chase the process or navigate a maze to get a family in the door.

Visible Progress

You stay informed

We keep you in the loop—including receipt of referral, fit and priority, and onboarding—so you are never left wondering where the family landed or what happened next.

Continuity Across Care

One waitlist, not four

Once a family is with Aspire, they can move across crisis, community, maintenance, and counseling without re-queuing or changing agencies. Fewer handoffs mean fewer gaps.

Coordinated, Not Competitive

We respect your role

You are coordinating a system; Aspire is one part of it. We coordinate around the plan you are accountable for rather than working at cross-purposes.

Coordination Around the System

We work within the plan already in motion

Aspire coordinates with schools around the IEP and with Regional Centers and health plans around eligibility and authorization. The goal is not to create another disconnected plan. It is to help the existing system hold together around the family.

What Happens After a Referral

From referral to a clear next step

Aspire keeps the process visible from the moment a referral is received through assessment and the beginning of services.

Acknowledgment

We confirm the referral

Aspire acknowledges the referral within two business days so you know it has reached the intake team and is moving forward.

You know the referral was received.

Intake & Fit

We contact the family

Our intake team reaches out to the family, confirms the funding path, and has a direct conversation about fit. If Aspire is not the right provider, we say so early.

The family gets a clear answer about fit and next steps.

Assessment

We build the starting plan

When Aspire is the right fit, we request authorization for an assessment, complete the assessment with the family, and review the findings with them before the plan goes to the funder.

Care begins from an assessment rather than guesswork.

Services Begin

We keep you informed

Once the assessment and authorization process is complete, services begin at the appropriate starting point and Aspire lets the referring partner know care is underway.

You know when the referral has moved into active care.

The referral should never disappear into a process. Aspire acknowledges referrals within two business days and keeps the path clear as intake, assessment, and services move forward.

Why Continuity Matters

The cost of a gap reaches the whole system

When care falls between systems, the impact is not only clinical. A failed transition can restart the entire process while the family, referring partner, and funding system absorb what happens in the gap.

When Care Resets

Every failed transition creates another starting line

Even when every organization involved is working within real constraints, an interrupted transition can undo momentum and force the system to rebuild context it already had.

A new intake

History must be gathered again, and the family retells a story the system may already know.

A fresh waitlist

The next provider begins from its own queue while support and momentum remain uncertain.

Progress exposed to the gap

Skills, routines, and stability may weaken before the next plan is fully in place.

When Care Stays Connected

The next level builds on what has already been learned

Aspire is structured so a family can move as needs change without discarding the records, relationships, and clinical understanding developed in the previous setting.

History travels forward

The next phase begins with context rather than rebuilding it from the beginning.

The plan changes without disappearing

Support can become more intensive or lighter while remaining connected to one clinical understanding.

Gains carry into the next setting

Progress made in one environment becomes the foundation for what happens next.

The System Rationale

Continuity interrupts the reset cycle

The crises that follow a failed transition can be among the most disruptive outcomes in the system. Aspire is built to hold continuity as needs change, so gains made in one setting carry into the next instead of resetting. That means fewer gaps, fewer failed placements, and fewer avoidable crises—for the family and for the system responsible for supporting them.

Start the Referral

Make a referral

Tell us who you are referring, what is happening now, and what support the current system is seeking. You do not need to determine the correct Aspire service before submitting the referral. Our team will review fit, urgency, and the most appropriate starting point.

Not sure whether the case fits? Submit the referral with the context you have. Aspire can help determine the next step, even when another provider or resource would be more appropriate.
Use this form for the information needed for an initial referral. Please do not include medical records, detailed clinical documents, Social Security numbers, or other highly sensitive information here. If supporting records such as relevant reports or IEP documents are needed, Aspire can coordinate those separately during the referral process.
